I understand that owning a pet is a long-term commitment that requires mature
consideration. I assert that I have the financial means to care for this cat(s) and will
provide a loving home and adequate medical care. I give permission to call my veterinarian
for a reference. I realize that the cat will take some time to get used to me and to my
household, and I will not hesitate to ask for advice from my veterinarian or from a
volunteer. I UNDERSTAND THAT SOMETIMES AN ADOPTION IS NOT A GOOD MATCH. IF I
DETERMINE THIS TO BE SO, I AGREE TO RETURN THE CAT TO ITS PREVIOUS HOME AT ANY
TIME.
I understand that a volunteer may contact me after the adoption. I agree to surrender the
cat if any of the information above is found to be false or if, in the judgment of the
volunteer, I am not maintaining the cat’s health or happiness by providing food, shelter, and
medical care, including getting it spayed/neutered as soon as medically safe and vaccinated
routinely. I agree I will not allow a female cat to have kittens before being spayed.
